Students who are enrolled in or have completed the UW Flexible Option Substance Use Disorders Counselor Certificate program who are interested in seeking Wisconsin state Substance Abuse Counselors (SAC) or Substance Abuse Counselor In Training (SAC-IT) certification should email Form #3079 to firstname.lastname@example.org after completing the top portion of the application. You will receive a confirmation email indicating you have completed your work with a tracking link.
You must fill out two separate copies of form #3079; one for the SAC-IT and one for the SAC completion.
Please also include the electronic documentation you received from UW Flexible Option indicating you’ve met the SAC-IT educational requirements or completed the program in full. Upon verification that the educational requirements have been met, your instructor or a designee from the University of Wisconsin-Madison Division of Continuing Studies will sign and submit the form directly to the Department of Safety and Professional Services (DSPS).
