The Substance Use Disorder Counseling Certificate satisfies the 360 hours of specialized education required to earn your SAC/C-SAC license, as well as the 100 hours required for your SAC-IT .
Complete the following steps when you are ready to apply for your license:
- Familiarize yourself with the steps to obtain a SAC-IT/SAC/C-SAC license. You may also want to review Wisconsin Chapter SPS 161 to better understand the statute that governs this process.
- If you do not already have a LicensE account with the Department of Safety and Professional Services, you will first need to set one up to receive a Wisconsin Application number (PAR-000XXXXXXX). Your Wisconsin Application Number will be needed in the next step.
- Complete the online #3079 form and list flexsuds@dcs.wisc.edu for the school contact.
- The UW-Madison Division of Continuing Studies will sign and submit your 3079 form directly to DSPS along with your completion certificate.
- Additional application steps beyond these educational hours are required to become fully licensed. Contact DSPS directly for more information.
Note: Upon receipt, the Department of Safety and Professional Services for the State of Wisconsin will process the application. Please be aware processing time may be lengthy. Students are encouraged to be proactive and check on the status of their application here.
