Return to Success Application

The Return to Success Scholarship is available to degree-seeking students returning to programs offered by UW Flexible Option. For complete details on eligibility and distribution requirements, please visit the Return to Success Scholarship page. The amount of the scholarship, which will be split over two consecutive subscription periods, is $3,000.

Submission of this form is required for consideration; however, receiving a scholarship is not guaranteed, as awards are subject to available funding. Students can expect an email from UW Flexible Option two weeks before the start of their intended return semester with further instructions.
