Sending Transcripts

Transcript Processing for Admissions to the UW Flexible Option

  1. You are required to submit official transcripts from each college or university attended.  Official high school transcripts are also required from the secondary school attended.  All students who completed an HSED or GED program must order transcripts through the Department of Public Instruction (DPI).
  2. Official transcripts should be sent electronically from each institution and sent to the University of Wisconsin Flexible Option. If your institution does not offer electronic submission, they may send transcripts in a sealed envelope sent directly from the institution sent to the UW Flexible Option Campus Office of Admissions.  DO NOT send any transcripts to UW-Milwaukee or UW-Parkside.
    • If you submitted a college/university transcript along with your UW System application, the transcript is considered unofficial.  We will not make admissions decisions using unofficial transcripts.
    • Applicants to the Diagnostic Imaging program who graduated from a hospital-based training program are not required to submit transcripts.  However, transcripts are required for graduates of an associate degree program.
    •  Transcripts are also not required for Special student applicants unless you intend to register for a course that has a prerequisite.

Please Note:

    1. Neglecting to report all institutions attended, even if you don’t want credit to transfer or no credit was earned, is considered to falsify your academic record and may jeopardize your chances of being admitted. You may be dismissed from the program, and admissions rescinded if the information is revealed later.
    2. The admissions application, and all supporting documents, are the property of the University of Wisconsin System.  All records submitted as part of the admissions process are permanent records of the university and are not released back to the student.

Process for Ordering Transcript

Ordering College Transcripts

Previous institutions from which you may transfer credit must be accredited by a regional or national accrediting organization recognized by the CHEA (Council for Higher Education Accreditation). This includes an Associate Degree in Nursing or an undergraduate nursing diploma.

  • It is always recommended to order electronic (PDF) transcripts, as physical (mailed) copies may experience delays from campuses.

When ordering PDF transcripts from your prior institution(s):

  1. Select the “University of Wisconsin Flexible Option” as the recipient institution
  2. If your order also requires an email address, use admissions@uwex.wisconsin.edu
  3. DO NOT send transcripts directly to UW-Milwaukee or UW-Parkside
    • Reentry Students – If you were previously enrolled in a UW Flexible Option program and now wish to return, review the returning student admissions process.
    • International Transcripts – If you completed college coursework at a foreign institution, the courses may be eligible for transfer credit. You will be asked to send your transcripts through an evaluation process with a foreign credential service, such as Educational Credential Evaluators or World Education Services, to determine U.S. credit equivalents.
    • Credit by Exam and Military Credit – You may use certain exams to transfer credit towards your degree. These include College Level Examination Program (CLEP), Advanced Placement (AP), International Baccalaureate (IB), or others.  Learn more about how exams and military coursework might be applied to your degree plan.
Ordering High School Transcripts or GED/HSED Scores

High School Transcripts or GED/HSED Scores are required unless you have already earned a bachelor’s degree.   It is recommended that you request an electronic official transcript be sent to the UW Flexible Option Admissions Office, if that ordering option is available, as physical (mailed) requests could experience some delays. 

When ordering PDF transcripts from your High School:

  1. If using Parchment or Credentials to place your transcript order, select the”University of Wisconsin UW Flexible Option” as the receiving institution.  If your order is through the National Student Clearinghouse, then select the University of Wisconsin Flexible Option as the receiving institution (see additional information on how to place an order under the section — Transcript Services).
  2. If your order requires an email address, use admissions@uwex.wisconsin.edu
  3. DO NOT send transcripts directly to UW-Milwaukee or UW-Parkside

While electronic transcripts are preferred, if you request paper transcripts sent by mail, use the following mailing address when placing your order:

Transcript Services

The following section includes information on third-party transcript services that are used by most colleges and universities.  You should also visit the website of your prior campus(es) Registrar’s Office for specific details on ordering instructions from that institution.

Parchment/Credential Transcript Services
  1. Create your account, choose your previous institutions, and click the Consent & Request button. This will take you to the Order page.
  2. Underneath the Order button, you will see a plus sign and the words: “Add Another School or Organization You Attended.” This will bring up a screen where you can manually type in these exact words: Flexible Option.
  3. Hit the Search button, and select UW Flexible Option.
  4. When using Parchment/Credentials to place your transcript order, you should select the “University of Wisconsin Flexible Option” as the receiving institution.

     Next Steps:

    • Electronically sign a consent form
    • Pay for the transcript
    • Receive an order confirmation

DO NOT send transcripts directly to UW-Milwaukee or UW-Parkside.

National Student Clearinghouse Transcript Services
  1. Create your account, choose your previous institutions, and enter your personal information.
  2. You will then be directed to a page where you can enter recipient details and you will input the following information:
    • Select a State: Wisconsin
    • School Name: Select University of Wisconsin Flexible Option
    • Transcript Purpose: Admission
    • Delivery Method: Electronic PDF (if available; if not, you will need to send transcripts by mail to our office address)
    • Check the box below
    • Recipient Email: admissions@uwex.wisconsin.edu
    • Name of Recipient: UW Flexible Option (this should be automatically filled)
    • Quantity: 1 copy
    • Processing Option: Now

     Next Steps:

    • Pay for the order by credit card
    • Electronically sign the consent form
    • Receive an order confirmation

DO NOT send transcripts directly to UW-Milwaukee or UW-Parkside
