UW Flexible Option Transfer Form

This form is for students enrolled in a University of Wisconsin-Milwaukee degree or certificate program seeking to transfer between UW Flexible Option and its campus online or in-person program. A transfer may be temporary or permanent. A student may wish to “step out” of UW Flexible Option to complete a course in a more traditional format and then return to UW Flexible Option. Or, a student may be pursuing a campus degree program and find that UW Flexible Option is a better fit and seek to permanently transfer to UW Flexible Option, or vice versa. All other students seeking to transfer to the UW Flexible Option or another institution should submit a new application to the desired institution.
